fix(tekton/v1): fix trigger `pr-to-feature-branches-labeled-with-lgtm` (#3860)

trigger on LGTM or unlabeled approved

- Extend trigger filter to fire for PRs labeled 'lgtm' or when
'approved' is removed.
- Bump helper image and add a guard to only add 'approved' if the PR
currently has the 'lgtm' label
